Sales & Use Tax Unit Exemption Certificate

If your business is tax-exempt or purchasing products from Delivered by Decade for resale, we are required to have a completed and signed Sales and Use Tax Unit Exemption Certificate on file before we can process your order without sales tax.

How to Submit Your Certificate

  1. Download the Ohio Sales and Use Tax Unit Exemption Certificate
    Download Here

  2. Complete all required fields, including:

    • Your business name and address

    • Type of business

    • Vendor information (Delivered by Decade)

    • Reason for claiming exemption

    • Your signature and title

  3. Email the completed form to:

If your business is based outside Ohio and you have a valid multi-state or out-of-state exemption certificate, you may submit that instead.

Important Reminders

  • Your tax-exempt status must be verified before sales tax can be removed from your order.

  • Exemptions only apply to qualifying purchases for resale or exempt use.

  • Orders placed before we receive and process your certificate will include applicable sales tax.
